Dehiscence of jugular bulb is a normal venous variant in which a superolateral extension of the jugular bulb extends into the middle ear cavity through a dehiscent sigmoid plate (arrow in A–C). On otoscopy this is often seen as a blue mass behind an intact tympanic membrane that distends on valsalva or ipsilateral jugular vein compression.
