A transverse otic-violating temporal bone fracture (white arrow) extending across the vestibule, with pneumolabyrinth in the vestibule (yellow arrow), is in keeping with the diagnosis of perilymphatic fistula. Perilymphatic fistula is due to abnormal communication between the middle and inner ears. It can be posttraumatic, postoperative, secondary to cholesteatoma, or idiopathic. Patients present with sensorineural hearing loss, tinnitus, vertigo, and recurrent meningitis.
