Sjogren Syndrome is a chronic systemic autoimmune disorder characterized by chronic lymphocytic infiltration causing salivary and lacrimal gland tissue destruction. Imaging appearance is stage-dependent within the parotid, and progresses from diffuse small cysts (“miliary pattern”, yellow arrows) with glandular swelling to multiple cystic (red arrows) and solid lesions, and then to fatty replacement and diffuse atrophy. Contrast enhancement is not seen.
